However, soft peel is performed without anesthetic ointment and causes more severe pain than laser toning due to the skin irritation occurring when the carbon lotion explodes, and furthermore, requires frequent repeated procedures which is burdensome.

Therefore, the preferred method is to alternate laser toning and soft peel every 2 weeks or to treat the patients with severe superficial freckles intermittently when the freckles are treated.

Laser toning, as it is well known, selectively destroys and releases melanin and melanosome in the cell to improve pigmentation, decreases melanocyte activity, slows down melanin production, and shrinks the dendrites in melanocytes, preventing the delivery of melanosome to epidermal cells (Figure 1).

The Genesis technique using the Long Pulsed 1064nm Nd:YAG Laser does not destroy the melanosome directly, but the energy absorbed by the water, and the second chromophore, and the melanosome is converted into thermal energy, causing thermal diffusion to the surrounding tissues and activating the fibroblast to improve the ECM environment.

At this time, the turnover rate of the epidermal cell layer is increased to reduce the pigment layer, thus improving the pigment and skin tone.

Similar effect can be created by using the quasi long pulse mode of the Q-switched 1064nm Nd:YAG Laser, although the output is weak.
