    Hello everyone. Im Koh Ik-soo, the president of APAS. This lecture will be an opportunity to look into the areas for filler injection and to understand step-by-step process of developing filler technique.

     

     

    Filler can be applied to everywhere on the face. Filling in hollow areas on the face is the basis of filler injection. The first thing to be considered in a filler procedure is accurate indication. It is very important to determine what type of patients can benefit most from filler injection, whether a certain patient would benefit more from a filler injection, prosthetic material graft, or bone shaving.

    In my opinion, the most important thing in filler injection is postprocedural change with time. Understanding the change is essential for a more comfortable filler procedure. In this context, doctors should not jump to a conclusion after a single injection of filler, but they always have to think about what to expect in the end. This means that doctors should consider the need for filler injections, not just to the depressed area, but also to the surrounding area and overall face as well.

    Now, lets see the key points of filler injection for each area.

    Forehead and Temples

    The forehead and temples can be considered in pair. The first key point for procedure on the forehead is to inject filler continuously. The most common complaint among patients after filler injection in the forehead is unevenness of the procedural area, mostly because fillers were not injected continuously. Doctors need to master the skill for continuous injection of filler on the forehead. The second is making the forehead generally smooth, which is a technique that need sufficient training. The third is to inject filler with a toxin to prevent the influence of muscle. The filler for the forehead should be highly water containing power and smooth. Lastly, patients should be informed in advance of possible change in the shape from pressure, such as lying on the front or hat.

    Hollow eyes and dark circles

    The first thing to consider for hollow eyes and dark circles is to differentiate superficial depression from deep depression. After filler injection for superficial skin depression, the folded area should be adjusted and the overall area should be lifted. The second point is to inject filler accurately along the curvature on the surface. Patients will complain of unevenness if the injection goes slightly out of the way. As was the case in the forehead, the injection should be made continuously. Both doctors and patients should be well aware of the fact that the filler may show through the thin skin.

    Aegyo-sal (youthful lower eyelid fullness)

    Whats most important is to inject filler only to the upper area of the inferior palpebral groove. Injecting more than 0.1cc makes puffy eyes as if those after crying. Some may even complain that the eyes look as if beaten. Another important point is to inject filler close to the eyelashes or the Aegyo-sal will look disconnected from the eyes. Of course the injections should be continuous, and patients should be informed that the filler may show through thin skin.
