Fursultiamine IV Therapy(Garlic injection)
Fursultiamine is a derivative of vitamin B1(thiamine). It enhances the nervous system and muscular system and decreases liver metabolism, which makes it safe in patients with liver dysfunction. It earned the nickname “garlic injection” due to the phenomenon of garlic scent remaining in the nose after treatment. Fursultiamine can help lactic acid excretion in athletes with a high level of daily activity and those in high-exertion professions. In particular, vitamin B1 is involved in glycolysis and TCA cycle to aid energy production. Moreover, it accelerates the pentose phosphate pathway to promote production of DNA, RAN and coenzymes. Similar to licorice therapy, garlic injection can be combined with other ingredients. However, the garlic injection can cause angiodynia and caution should be given in dose setting. Generally, ½ ampoule can be safely combined in 200-250ml base solution and 1 ampoule in 500ml solution. I combine licorice and fursultiamine IV therapies in patients with fatigue.
Placenta injection
Generally, the placenta injection is regarded as the most affordable stem cell therapy. It has endlessly varying effects. Cancer cells with very rapid differentiation take 3-4 years to grow to be 3kg. On the other hand, it takes a fertilized human egg only 10 months to grow into a full human weighing 3kg. There are two types of placenta IV therapies. Placenta extracts known as melsmon uses the chorion degraded by hydrochloric acid hydrolysis(similar to digestion in the gastric tract) for breaking down the protein into the smallest particles. It is also rich in various minerals and vitamins found in the placenta. It contains benzyl alcohol which works as a mild anesthetic during subcutaneous injection. Therefore, the placenta injection must not be given intravenously. The low-molecular nutrients are particularly beneficial in elderly patients. On the other hand, placenta hydrolysate known as laenec contains chorion and umbilical cord and goes through two phases of processing. First, enzymatic modification and molecular fractionation filter high-molecular proteins such as growth factors or cytokines(interleukin). Second, hydrochloric acid hydrolysis divides the remnants into low-molecular amino acids and various minerals and vitamins. Laenec injections contain less low-molecular nutrients but are rich in growth factors and cytokines which brings excellent tissue regeneration and anti-inflammatory effects. It is can also be safely used in IVNT. I currently use 3-5 ampoules for a single treatment of placenta injection. Placenta has varying uses as a local anesthetic or topical application for aesthetic improvement.

Glutathione(White jade injection)
Glutathione therapy is well-known for its whitening effects and is nicknamed “white jade injection.” However, its efficacy in the skin is only 10%. Its main action is detoxification and anti-oxidation of the nervous system, lungs and liver. Glutathione is a combination of three amino acids; glutamate, cysteine and glycine. None of the three are essential amino acids but cysteine comes from degradation of methionine, an essential amino acid. Cysteine(NAC, N-acetyl-cysteine) is combined to increase the bioavailability of glutathione. NAC include Muteran and Flumucil capsules, well-known antitussives and expectorants. Glutathione acts with glutathione peroxidase(GSHPx) to convert hydrogen peroxide(H2O2) generated in the human body into water(H2O) and oxygen(O2). Without glutathione, hydrogen peroxide is converted into even more potent reactive oxygen, hydroxylgroup(HO•) by iron(Fe2+). Hydroxyl group causes oxidative damage powerful enough to induce genetic mutation. Many toxic anti-cancer medications use hydroxyl group to destroy cancer genes. And normal cells are also damaged through this mechanism. The anti-oxidative action against hydrogen peroxide continuously occurs throughout our body in the extracellular and intracellular fluids and cell membranes. This is why glutathione is also known as a broad range anti-oxidant. One vial of glutathione is usually 600mg, but recently 1,200mg vials are being introduced. In general, 1-3V(600mg-1,800mg) are mixed in saline solution of 40-80ml for slow IV infusion or rapid drip infusion. As it oxidizes quickly, do not delay for over 15 minutes after opening the vial. Glutathione’s use is increased due to reports of its benefits in Parkinson’s disease. In Parkinson’s disease, 5-7V(3,000mg-4,200mg) are used twice weekly for at least a month.
Lipoic acid(alpha lipoic acid, thioctsan, Cinderella injection)
Lipoic acid exists in R and S types and only the R type is needed in the human body. Unlike other antioxidants, lipoic acid is available in the oxidized form. Once oxidized lipoic acid is administered, it is immediately absorbed into cells to bind with two hydrogens and becomes the reduced, complete form(DHLA, dihydrolipoic acid). DHLA is a potent antioxidant that travels into both hydrophilic and hydrophobic tissues. It is used in diabetics with peripheral nerve disorders to relieve neural symptoms. It is also involved in glycolysis, TCA circuit and energy production along with VitaminB1(garlic injection). It revives other antioxidants to bring their beneficial action again. It is available in a 25mg vial. Typically, 1-3 V are added in saline solution of 100ml glucose and administered in the maximal speed. However, patients with vitamin B1 deficiency can experience nausea or vomiting after lipoic acid injection. Therefore, I recommend prescribing lipoic acid injection along with garlic injection.
Five essential antioxidant and mineral injections
The five essential nutrients are selenium, zinc, copper, manganese and chromium. Selenium(200μg) is a mineral that is an antioxidant and anti-cancer medication. It has various actions including cancer prevention, immune support, elimination of heavy metals, and fertility enhancement, etc. Zinc promotes release of growth hormones, strengthens male stamina, and immune support. It was also recently found to be beneficial in diabetes. Copper helps maintain the elasticity of cardiovascular walls and promotes fat metabolism. Manganese promotes collagenesis and prevents dermatitis. Chromium is involved in glycometabolism and maintenance of peripheral nerve functions. For safety, minerals are usually added to amino acid solution(100ml or 250ml). Mineral injection was recently reported to reduce edema and accelerate wound healing after surgery. This injection causes pain during treatment and increasing the amino acid solution to 250ml can be more tolerable in patients sensitive to pain. Pain tends to subside with repeated treatments and it is important to ensure patients that next treatments will be easier.
In this article, we took a quick look at micronutrient therapies that are widely used in private practices. Separate use, appropriate combinations or consecutive delivery of two or three therapies all have benefits. There are no set standards for nutrition therapy.
