    Q-switched Nd:YAGlaser has most of thephotothermal effect and some of the photomechanical effect and it is difficult to precisely irradiate on the target. On the other hand, the picosecond laser mainly delivers photomechanical effect which enables effective tattoo removal. It selectively destroys tattoo ink particles with high precision. However, as the picosecond laser device is very costly, it is difficult for a private practitioner to purchase the device just for tattoo removal. The picosecond laser is not yet a standard treatment for other pigment removal procedures and the parameters for each lesion type are yet to be established. Another problem is that it may not be as safe as previous nanosecond lasers in pigment treatments. There is heated debate between clinicians over whether the long-pulsed alexandrite or the picosecond laser is better for pigment removal.

     

    Lasers with longer pulse duration than the nanosecond laser may cause a higher risk of PIH mainly because thermal effect takes place in tissues after exposure to laser which leads to cracks and surface debris. On the other hand, lasers with pulse duration shorter than the picosecond lasers have strong photothermal effect to cause selective LIOB within skin tissues which leads to plasma and cavity formation. This minimizes the thermal effect on surrounding tissues and lowers the risk of PIH through limiting creating of cracks and surface debris.

     

    Long-pulsed laser can easily stimulate the dermis regardless of the lasing medium and requires aahigh level of experience on the part of the clinician due to the higher risk of side effects. This is why in the end, the picosecond laser will be favored.

     

    Skin rejuvenation: 755nm picosecond laser is the answer.

     

    To achieve pore reduction and smoother skin texture, the fractional laser, needle RF, or contact type RFare used. The CO₂fractional laser may be more effective in pore reduction but is painful and has long downtime with a higher risk of side effects. The Needle RF devices vary in terms of the needle type, needling technique, RFstrength and duration. However, as this is an invasive therapy, it causes a lot of pain, mild bleeding and even needle marks after treatment. The efficacy and safety of contact type RF device sits somewhere between the fractional laser and needle RF but the cost is very high and takes a long time for the desired outcome to show.

    The photomechanical effect needs to be maximized in pigment and tattoo removal. In this area, the picosecond level pulse duration is very important but the wavelength has to be correctly chosen to minimize side effects in skin rejuvenation. As we know, longer wavelengths have deeper delivery in the skin. The 755nmwavelength is more effective in pore tightening than 1064nm.

     

    Fractionation of picosecond laser beam uses a more advanced lens processing technique compared to nanosecond laser to improve energy efficiency. Usually, the micro lens array (MLA) is added to the handpiece for fractional treatments but MLA increases light transmittance and improves light absorption in tissues. This enables overall rejuvenation with pore tightening and skin tone improvement.
